Post Title: Hospice Palliative Care Clinical Nurse Specialist

Accountable to: Clinical Lead – Community Team

Type of Contract: Permanent

Hours: 37.5 hours per week worked on a rota basis inclusive of weekends and Bank Holidays
Salary WTE: £40,385

This post involves regular, direct unsupervised access to vulnerable adults and therefore is exempt from the rehabilitation of offenders Act 1974

We have an exciting opportunity for a forward thinking, enthusiastic and motivated registered nurse with substantial post registration experience to work within our hospice community team. You will provide the clinical leadership, caseload management and coordination necessary to ensure high quality, evidence based specialist and supportive palliative care is delivered with care, compassion and dignity, to patients and those that matter to them.

Whilst leading an internal team of HCNS Associates and support staff, work as a positive, pro-active role model and autonomous practitioner demonstrating excellent standards of specialist assessment, care planning and evaluation of interventions to ensure the needs and choices of people accessing hospice community services are met.

You will hold post registration qualifications in palliative care and/or a clinical field relevant to the role and be a non-medical prescriber or willing to undertake in post. With recent experience in the delivery of specialist palliative care you will have well established clinical assessment and advanced communication skills and the ability and experience to lead on the development and delivery of formal and informal education.
