Job Title: Health Care Assistant – Supportive Palliative Care (Hospice at Home)
Responsible to: Clinical Lead
Hours: Negotiable (minimum of 20 hours maximum 35 hours worked on a rota basis over 7 days inclusive of nights, days, evenings, weekends and Bank Holidays)
Contract type: Permanent subject to 6 month probationary period
Salary: £10.78 per hour

Job summary:
An exciting opportunity has arisen to work in our developing Hospice at Home service providing support in the community for individuals with life limiting illness. You will work supporting patients and families in their own homes, delivering high standards of holistic care to the patient and their families. Whilst promoting independence and quality of life and engaging in activity that is meaningful to patients and those that matter to them, you will support individuals to remain in their own home environment preventing avoidable admissions to hospital. You will work without direct supervision in accordance with care plans provided by registered practitioners.
Our service cover the communities of Blackburn with Darwen, Hyndburn and Ribble Valley and the ability to travel across these areas is essential. Applicants must be aware that the nature of this work means a commitment and ability to undertake a significant amount of night duty.
You will be required to positively engage with opportunities for development and maintenance of skills and knowledge including rotational placement within other clinical areas including for example the Inpatient Unit and Support and Wellbeing Service.
