East Lancs Hospice

East Lancashire Hospice is a registered charity, founded in 1984. We support and care for patients with a life limiting illness, their families and others close to them. We provide this in a number of ways; at the hospice and in people’s own homes across our community of Blackburn, Darwen, Hyndburn and the Ribble Valley.

With approximately 120 employees and over 250 volunteers, our people are our biggest asset whom we value tremendously. Our services to the local community would not be possible without our motivated and caring workforce who always strive to deliver excellence in all that they do. Senior Nurse – Community Supportive Care (Hospice at Home)

An exciting opportunity has arisen as part of new investment and ongoing service development for a motivated individual passionate about high quality palliative and end of life care to join our team.

You will have a key role in providing support to patients in their own home environment who have palliative and end of life care needs, role-modelling high quality care and clinical standards and promoting a learning environment for team member and other involved in the person’s care whilst supporting informal carers. You will be responsible for planning, assessing, implementing, and evaluating care in collaboration with the District Nurses Team in order to prevent avoidable hospital admission or support.

The Successful Candidate

Are you a registered nurse with excellent communication, organisational skills?

Are you able to assess, plan and evaluate the delivery of high quality care?

Are you able to work in a community setting with a sound understanding of the support needed to help people to remain in the own home environment where this is possible?

Do you have an understanding of the needs of informal carers?

Are you passionate about palliative and end of life care?

If so this role may be exactly what you are looking for.

What we offer

A salary range [dependant on qualifications and experience] of £36,671 – £41,154. Hours are worked on a rota basis of up to any 4/5 days of the 7-day week to meet service need inclusive of weekends and Bank Holidays.
